For good and valuable consideration, the adequacy of which is acknowledged and includes without limitation access to the seminars presented by Southern Anesthesia Services Inc. (“SASI”) with the understanding that SASI will invest substantial time and expense to produce certain audio and/or visual works in reliance upon this document, I hereby consent to participation in said production and to being personally identifiable in the resulting media, regardless of production method, including, without limitation, digital and analog photography and videography, and audio recordings (“Media”) whether captured or created by SASI directly, or by those engaged by SASI.

I completely and forever release any current and future financial interest in and any physical or intellectual control of the Media to SASI and its successors, assigns, and designees so that they may use the Media freely and at their discretion.

I authorize the universal and unrestricted reproduction of my voice and/or likeness in connection with the use or manipulation of the Media by SASI, its successors, assigns, and designees, and those acting with SASI’s permission and authority, in any form now existing or yet to be invented, for all purposes, including, without limitation, transmission, advertising, art, promotion, solicitation, or trade. Additionally, I waive any right to approve the eventual use or purpose to which the Media may be applied; or to inspect or approve the finished images, videos, advertising copy, artwork, materials, or other audio or visual matter resulting from the Media’s implementation, use, or manipulation, whether in its original form or processed, altered, blurred, distorted, combined with, or used in conjunction with any other media, and will hold SASI, its successors, assigns, and designees harmless and indemnify same against any claims or demands arising therefrom.

SASI is not responsible for acts or omissions of speaker, session attendees, or session venue operator/owner and makes no representations or warranties regarding speaker, session attendees, or session venue operator/owner except as specified herein, if any. Sasi does not accept liability, including any liability for lost profits, loss of business, or other consequential, special, indirect, or punitive damages even if advised of the possibility of such damages or for any claim by any third party except as expressly provided herein. I agree that sasi is not liable or responsible for any amount of damages above the aggregate dollar amount paid by me or on my behalf to sasi for seminar only, which does not include travel, accomodations, meals, or other amounts that are not received ultimately by sasi, but rather by a third party provider, as reasonably demonstrated in writing by sasi. I acknowledge that sasi would not enter into this agreement without these limitations on liability.

This agreement shall be governed by the laws of the state of South Carolina. I am not relying on any warranty, guarantee, or representation, whether oral or written, by SASI. By signing this release, I affirm that I am of the age of majority and have the right to contract in my own name. I have read this entire document, understand its contents, and agree to the rights and responsibilities it defines. This agreement shall be binding upon me and my heirs, legal representatives, and assigns.
